Job Title: Nurse Navigator, Ross Heart Hospital (Remote)

Department: Ross | Heart Failure Transitions of Care

Scope of Position/Position Summary:

The Nurse Navigator improves outcomes by reducing all-cause readmissions and coordinating episodes of care among patients in a defined population or disease process. The primary focuses are:

  • Coordination and delivery of efficient, effective, compassionate, patient-centered care and safe transitions across the care continuum.
  • Providing care navigation services to the cardiac population.

The Nurse Navigator monitors patient care and physiological needs; educates, documents, communicates, and collects data to evaluate patient/family readiness for discharge. They facilitate lateral integration of the care team, collaborating with physicians, nurses, and staff across the continuum to provide comprehensive disease management, assessment, treatment, education, and follow-up. The Nurse Navigator also communicates the care plan in coordination with pre- and post-acute care settings and serves as a liaison with post-acute partners as determined by the Heart Leadership team. The role involves managing patients and facilitating care navigation primarily through telephone calls in a post-acute setting.

Minimum Qualifications:

For Hire: Registered Nurse required. Five years of recent clinical experience applicable to the population is preferred; knowledge of evidence-based practice and disease-specific processes; familiarity with technology and payer protocols is desired. Experience in case management and post-acute care (home care, SNF, LTACH) is desirable.
