Job summary

Trinity Health is seeking a Nurse Senior - Navigator to oversee care for newly diagnosed cancer patients across five cancer centers. The role emphasizes coordination across Surgery, Medical Oncology, Radiation Oncology, Genetic Counseling and Research.

Qualifications

• Current Michigan Nursing License.
• Graduation from accredited nursing program: BSN Required.
• Three or more years of progressive clinical nursing experience in Cancer, Thoracic, Women’s Health, or related field.

Responsibilities

• Identifies patients in need of ongoing diagnostic interventions and related services; facilitates initial encounter in multidisciplinary clinic.
• Develops and manages monitoring systems to help meet quality initiatives.
• Monitors and tracks all patients for whom biopsies are recommended; ensures timely biopsy scheduling, patient education and support, and referrals.
• Tracks biopsy results and follows patients with positive biopsies to expedite definitive treatment and referrals to specialists.
• Organizes and actively participates in regular multidisciplinary care conferences (Breast and Lung).
• Provides initial genetic screening and cancer risk assessment, coordinates referrals for further evaluation.
• Provides input to Breast and Lung Advisory Committees for quality assurance programs.

What You Will Do

• Identifies patients in need of ongoing diagnostic interventions and related services; and facilitates initial encounter in multidisciplinary clinic.

• Monitors and tracks all patients for whom biopsies are recommended: nsures timely biopsy scheduling, comprehensive patient education and support, appropriate assessment and referrals, follow-up on abnormal results, and continuity of care to prevent patients from being lost to follow-up.
• Tracks biopsy results and follows all patients with positive biopsies to expedite definitive treatment and ensure that referrals to all necessary specialists (Radiation Oncology, Medical Oncology, Plastic Surgery) are accomplished in a timely manner.
• Organizes and actively participates in a regular multidisciplinary care conference, (Breast and Lung)
• Provides initial genetic screening and cancer risk assessment, responds to all genetic screening inquiries, and coordinates referrals to appropriate internal or external resources for further evaluation based on established criteria.
• Provides input to the Breast Advisory and Lung Advisory Committees in the development of quality assurance programs that include tracking information, care conference presentations, outcome measures and patient satisfaction indices.
