Job summary

Trinity Health MI in Michigan is seeking a Nurse Senior - Navigator to support newly diagnosed cancer patients across five cancer centers. Navigators coordinate care across Surgery, Med Onc, Rad Onc, Genetic Counseling and Research to ensure timely, coordinated treatment.

This full-time benefited position schedules 32 hours per week (0.8 FTE). BSN and active Michigan nursing license are required, with 3+ years cancer care experience preferred.

Qualifications

  • BSN degree or higher and active Michigan nursing license.
  • Minimum 3+ years progressive clinical nursing experience in cancer care.
  • Strong ability to coordinate multidisciplinary care.

Responsibilities

  • Identifies patients in need of diagnostic interventions and facilitates initial multidisciplinary encounters.
  • Develops and manages monitoring systems to support quality initiatives.
  • Monitors and tracks patients with biopsy recommendations and ensures timely scheduling, education, referrals, and follow-up.
  • Traces biopsy results and coordinates timely referrals to Radiation Oncology, Medical Oncology, and Plastic Surgery.
  • Organizes and participates in regular multidisciplinary care conferences (Breast and Lung).
  • Provides initial genetic screening and risk assessment; coordinates referrals for further evaluation.
  • Contributes to quality assurance programs through data tracking and care conference presentations.

POSITION PURPOSE
  • The Nurse Senior - Navigator is responsible for newly diagnosed patients with cancer across our five cancer centers. Nurse Senior – Navigators are vital to the functioning of the cancer program. They coordinate care across disciplines of Surgery, Med Onc, Rad Onc, Genetic Counseling and Research.
  • Full-Time benefited position scheduled 32 hours per week (0.8 FTE)
Miniumum Qualifications
  • Current Michigan Nursing License
  • Graduation from accredited nursing program: BSN Required.
  • Three or more years of progressive clinical nursing experience in Cancer, Thoracic, Women’s Health, or related field.
What You Will Do
  • Identifies patients in need of ongoing diagnostic interventions and related services; and facilitates initial encounter in multidisciplinary clinic.
  • Develops and manages monitoring systems to help meet quality initiatives.
  • Monitors and tracks all patients for whom biopsies are recommended: nsures timely biopsy scheduling, comprehensive patient education and support, appropriate assessment and referrals, follow-up on abnormal results, and continuity of care to prevent patients from being lost to follow-up.
  • Tracks biopsy results and follows all patients with positive biopsies to expedite definitive treatment and ensure that referrals to all necessary specialists (Radiation Oncology, Medical Oncology, Plastic Surgery) are accomplished in a timely manner.
  • Organizes and actively participates in a regular multidisciplinary care conference, (Breast and Lung)
  • Provides initial genetic screening and cancer risk assessment, responds to all genetic screening inquiries, and coordinates referrals to appropriate internal or external resources for further evaluation based on established criteria.
  • Provides input to the Breast Advisory and Lung Advisory Committees in the development of quality assurance programs that include tracking information, care conference presentations, outcome measures and patient satisfaction indices.
