Job summary

Saint Joseph Hospital - Elgin is seeking a Nurse Navigator to coordinate patient care across the health system. You will help patients overcome access barriers, connect with resources, and provide education while building strong relationships with service providers and the community.

The role includes assessing patients' physical, psychological, and social needs to improve outcomes, satisfaction, and reduce overall costs.

Qualifications

• Current and valid state RN License.
• Current BLS certificate upon hire and maintain current.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of two (2) years of nursing experience.

Responsibilities

• Coordinate service and identified patients through the health system, assisting with access issues and identifying resources.
• Provide educational material and develop relationships with service providers and the community.
• Assess physical, psychological, and social needs to enhance outcomes, satisfaction, and cost efficiency.
• Engage in community education, screening, and monitoring outreach to promote survivorship.
