Job summary

Saint Joseph Hospital - Elgin is seeking a Nurse Navigator to coordinate patient care across the health system. You will help patients overcome access barriers, connect with resources, and provide education while building strong relationships with service providers and the community.

Qualifications

• Current and valid state RN License.

• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of two (2) years of nursing experience.

Responsibilities

The Nurse Navigator will be responsible for coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and community. The Nurse Navigator coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Nurse Navigator is a consistent care coordinator throughout the continuum of care and assess the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Nurse Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
