Job summary

Kettering Health Network is seeking a Nurse Navigator for Outpatient Services to guide patients and families through the care journey, coordinating education and services while ensuring timely access. This role acts as the primary contact across the care continuum, supporting patients with personalized navigation and coordination.

Qualifications

• ADN or BSN from an accredited institution.
• Minimum of 2 years of clinical nursing experience in an acute care or relevant specialty setting.
• Current RN license in the State of Ohio; BLS certification or ability to obtain within 90 days of hire.

Responsibilities & Requirements

The Nurse Navigator guides patients and families through the complexities of healthcare by coordinating care, providing education, and supporting timely access to services. This role serves as a consistent point of contact, ensuring patients receive comprehensive, patient-centered support throughout their care journey. Performs other duties as assigned. All job responsibilities are to be carried out in alignment with Kettering Health's mission, vision, values, and B Safe program.

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited institution
• Minimum of 2 years of clinical nursing experience in an acute care or relevant specialty setting
• Current Registered Nurse (RN) license in the State of Ohio; Basic Life Support (BLS) certification or ability to obtain within 90 days of hire

Preferred Qualifications

Bachelor of Science in Nursing (BSN) preferred Experience in care coordination, patient education, or case management; oncology experience preferred if applicable to patient population Certification in a relevant specialty (e.g., Oncology Certified Nurse, Nurse Navigator certification) preferred
