Job summary

Cooper University Health Care in Camden, New Jersey, is seeking a Nurse Navigator to join the Bone & Joint Institute multidisciplinary team. You will coordinate care across the orthopedic continuum, provide patient education, and support clinical teams.

Short Description

The Nurse Navigator is an integral member of the Bone & Joint Institute multidisciplinary team, working collaboratively with physicians and advanced practice providers to coordinate care across the orthopedic continuum. The Nurse Navigator provides patient education, clinical support, and care coordination; facilitates communication between patients, families, providers, and support services; identifies and addresses barriers to care; and promotes efficient care delivery.

Experience Required

Minimum 3-5 years of nursing experience required; orthopedic, musculoskeletal, surgical, or specialty practice experience preferred. Demonstrated expertise in patient education, care coordination, navigation, multidisciplinary collaboration, and clinical triage. Proficiency with EHR documentation.
