Summary

Works in partnership with physicians, hospital personnel, patients, significant others, and pre/post-acute services to educate, coordinate, and facilitate the care of patients with Heart Failure. Responsible for organizing and analyzing data supporting Heart Failure initiatives. Completes abstraction and data entry into required databases. Develops and generates outcomes reports and acts as a resource for members of the Division of St. Clair Medical Group Cardiology Heart Failure Services. Facilitates orientation of new staff and planning for service line events and new procedures.

Minimum Qualifications
  • Current licensure as a Registered Nurse in Pennsylvania.
  • Bachelor’s degree in nursing.
  • Three years of experience planning and implementing care for patients with heart failure and/or cardiac conditions.
  • Effective verbal and written communication skills.
  • Ability to address complex issues with patients, family members, and staff across multiple departments and community providers.
  • Ability to work independently and gather pertinent patient information.
Preferred Qualifications

Master’s degree in nursing or a health-related field.
