Job summary

Nuclear Care Partners is seeking a Nurse Navigator / Field Case Manager for the Las Vegas area. This hybrid role focuses on coordinating care, performing in-home assessments, and guiding patients and families through the care continuum.

The ideal candidate holds a BSN and Nevada RN licensure with prior case management or leadership experience. A valid driver's license is required, and the position offers weekday hours with some flexibility for weekend coverage.

Qualifications

• Active RN license in Nevada
• Experience in field case management or leadership roles preferred
• Strong professionalism and interpersonal skills
• Valid driver's license and in good standing

Responsibilities

• Perform routine face-to-face patient visits in the home setting for caseload
• Build rapport with patients and families to support care plans
• Coordinate with Clinical Director, supervisors, and care teams
• Manage care authorization with Authorization Specialist for new admissions
• Develop and implement individualized Plans of Care with physician orders
• Provide nursing assessments and progress notes
• Offer guidance and training to staff and home health aides
• Support patient advocacy and coordinate appointments or therapies
