Job summary

Mercy Orthopedic Hospital in Ozark, Missouri seeks a Nurse Navigator who guides patients across the continuum of care with clinical expertise. You will coordinate treatment plans, collaborate with physicians, families, and the care team to ensure seamless transitions and high-quality outcomes.

The ideal candidate has a nursing degree, an active RN license in Missouri, at least two years of nursing experience, and BLS certification. Advanced practice or MSN is preferred.

Qualifications

• Graduate from a school of nursing is required.
• Active RN license in Missouri is required.
• 2 years nursing experience is required.
• BLS certification required within 30 days of hire.

Responsibilities

• Coordinate patient care across the continuum with physicians, families, and the care team.
• Provide patient education and support throughout treatment and discharge.
• Support Mercy Signature Service standards and mission in daily practice.

The Nurse Navigator is nursing specialist who follows patients across the continuum of care. The Navigator coordinates with an advanced level of clinical expertise, all aspects of treatment and care for, and in collaboration with, Mercy's patients, their family and significant others, physicians, interdisciplinary team, and other support staff. This specialist performs duties and responsibilities in a manner consistent with our mission, values, and with Mercy Signature Service standards.

Minimum Requirements:

• Required Education: Graduate from a school of nursing is required.
• Preferred Education: Advanced Practice/master's degree in nursing is preferred.
• Required Experience: 2 years nursing experience is required.
• Preferred Experience: Formal patient education and community educational presentations
• Support group facilitation
• Licensure: Active RN (Registered Nurse) license in state of practice is required.
• Certifications: BLS (Basic Life Support) is required within 30 days of hire is required.
