Qualifications

• Registered Nurse licensure in Pennsylvania or eligibility via reciprocity.
• NJ RN licensure required for New Jersey locations.
• Bachelor's degree preferred.
• Three to five years specialty-specific experience preferred.

Responsibilities

• Serves as a bridge between patients and clinical / medical staff.
• Guides patients, family and caregivers through the healthcare process and coordinates a plan of care including appointments, transportation, education, and access to support services.
• Advocates on behalf of the patient.
• Ensures patients understand their disease process and treatment options.
• Acts as a liaison between clinical / medical staff and patients during treatments.
• Manages cross-continuum care of multidisciplinary patients to provide high quality care.
• Provides multidisciplinary team with patient summaries.
• Serves as a consistent contact for patient, family, and caregivers throughout diagnosis and treatment.
• Empowers patient/family to participate in the plan of care, documents concerns, monitors progress, and consults with physicians.
• Collaborates with clinical leadership per service line.
• Interacts with administration and facilitates program improvement; keeps team updated on progress and outcomes.
• Identifies patient/provider needs and ensures delivery of quality services.
• Other related duties as assigned.
