The Nurse Coordinator functions independently to coordinate care of patients. Responsible for planning, education and support of patient and family. Assures that the continuum of care process is complete, including intra-interdepartmental process to include: pre-admit, hospitalization, discharge and follow up in the OP. Maintains program data for analysis and RFPs. Works with Director and Medical Director / team for the development and growth of the program. Provides nursing services as needed. Other responsibilities as assigned.
