Job summary

HCA Healthcare in Nashville seeks a Nurse Navigator, an RN with advanced knowledge in cardiac care, to support outpatient heart failure clinics and inpatient care optimization. The role is full-time, Monday–Friday, day shift with no nights, weekends, holidays, or on-call.

The Nurse Navigator will operate telemetry monitors, educate patients, document treatments, and respond to emergencies following SAMC protocols. ACLS preferred and CHFN certification preferred.

Qualifications

  • Graduate of an approved school for registered nurses and current state required license to practice nursing.
  • ACLS certification preferred; BLS required.
  • Certification as a heart failure nurse (CHFN) preferred.
  • Minimal three years' RN experience with 1-2 years in cardiac patients.
  • Direct involvement in education and care of patients with coronary heart disease and heart failure.

Responsibilities

  • Assist daily operations of the outpatient heart failure clinic.
  • Document patient treatment accurately per policy.
  • Assist with inpatient heart failure care optimization.
  • Initiate ACLS measures in cardiac emergencies per SAMC protocols.
  • Provide education to patients.
  • Demonstrate communication and organizational skills and professional development.
  • Follow current standards of practice and protocols.
  • Perform duties in supervisor's absence.
  • Maintain flexible schedule to cover clinic needs.

Job Summary And Qualifications

Full time (Monday - Friday, day shift), no nights, no on-call, no weekends, no holidays

The Nurse Navigator is a registered nurse with RN who possesses advanced knowledge and skill in cardiac nursing. Makes decisions of moderate difficulty. Handles confidential information regarding patient records. Operates and analyzes telemetry cardiac monitors, exercise equipment and patient evaluation tools. May be required to make critical decisions concerning the initiative of emergency medical procedures.

What you will do in this role:
  • Assists in the daily operations of the outpatient heart failure clinic.
  • Documents patient treatment regularly and accurately according to departmental policy.
  • Assists with inpatient heart failure patient care optimization.
  • Initiates advanced cardiac life support measures according to SAMC protocols in the event of a cardiac emergency.
  • Provides appropriate education to all patients.
  • Demonstrates effective communication and organizational skills, and evidence of professional development.
  • Familiar with current standards of practice and clinical and inpatient protocols.

  • Performs miscellaneous job duties as assigned in the absence of the nurse manager or supervisor.
  • Maintains a flexible working schedule to allow for clinic coverage.
What Qualifications you will need:
EDUCATION:

Graduate of an approved school for registered nurses and current state required license to practice nursing.

Advanced cardiac life support (ACLS) certification preferred.

Certification as a heart failure nurse preferred (CHFN)

EXPERIENCE:

Minimal three years' experience as a RN with 1-2 years' experience working with cardiac patients, preferably on a telemetry unit or coronary care unit required.

Must have been directly involved in education and care of patients with coronary heart disease, including heart failure.

CERTIFICATION/LICENSE:

Current Registered Nurse license to practice nursing in the State of employment

BLS required

ACLS preferred per policy
