Job summary

Bethesda Group in Colorado Springs is seeking a Facility Medication Nurse Coordinator to ensure MAR alignment between PACE and assisted living facilities. You will update medication orders, ensure quarterly signings, and provide orders to facilities in a timely manner while coordinating with nursing staff.

Qualifications

• Graduate of an accredited LPN program and Colorado license required.
• One year of healthcare experience in hospital, long-term or home health care; elderly care preferred.
• Proficient with email, internet, electronic timekeeping and EHR.

Responsibilities

• Provides medication reconciliation and communicates with 20+ assisted living facilities biweekly.
• Rounds regularly in ALFs and interfaces with staff to improve safety and satisfaction.
• Manages MAR between PACE and facilities for 200+ participants.
• Ensures accurate implementation of orders written by PACE providers.
• Communicates with staff, families and caregivers about medications and treatments.
• Maintains inventory, handling of controlled substances, and monthly cart checks.
• Supports ongoing education and documentation in the EMR.
