Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will provide high-quality nursing care to our residents, ensuring their health and well-being while contributing to our mission of family-focused care.

This is a great opportunity for an LPN looking to start or continue their career in a dedicated and compassionate environment. Our team is committed to providing the best quality care for the residents at our community.

Starting pay for this position, based on experience, is $27 an hour.

The typical schedule for this opportunity is five (5) 8-hour shifts a week, including an every other weekend rotation. Specific schedules will be discussed during the interview.

Responsibilities
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient health and provide updates to the healthcare team
  • Assist in the development and implementation of individualized care plans
  • Perform assessments, including vital signs and medical history evaluations
  • Educate patients and families on health management and wellness practices
  • Coordinate with interdisciplinary teams to ensure comprehensive care delivery
  • Document patient progress accurately in electronic health records
