Job summary

Evercare of Breese is seeking Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join a caring, fast-paced skilled nursing team. You will provide direct resident care, administer medications, and contribute to care plans in collaboration with an interdisciplinary team.

Qualifications

• Minimum 1 year of experience preferred (new graduates are welcome to apply).
• Current and active Illinois LPN or RN license in good standing.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Provide direct resident care, including assessments, medications, and treatments.
• Develop, implement, and update individualized care plans in collaboration with the healthcare team.
• Perform ongoing assessments and evaluations to ensure quality outcomes.
• Communicate effectively with residents, families, and clinical staff to ensure continuity of care.
• Maintain accurate, timely, and complete documentation in accordance with facility policies and regulations.
• Participate in quality improvement and patient safety initiatives.
