Job summary

Optalis Healthcare (Health & Rehabilitation Centers) seeks a Licensed Practical Nurse to coordinate resident care and ensure compliant, high-quality service. You will collaborate with administration and clinical staff to implement care plans and respond to resident needs with professionalism.

Ideal candidates hold a valid LPN license, CPR certification, and a High School diploma; LTC experience is preferred.

Qualifications

• Possess a Licensed Practical Nurse license in good standing.
• Have a High School diploma or GED.
• Maintain CPR certification.
• Preferred: 2 years in long-term care management of resident care.

Responsibilities

• Coordinate resident care with Administrator and Director of Nursing and other disciplines.
• Ensure care plans are implemented and updated based on resident needs.
• Make effective clinical decisions to maximize outcomes for residents.
• Answer call lights promptly and communicate needs to the appropriate person.
• Provide daily rounds and concise shift reports to oncoming staff.
• Support survey readiness and quality assurance activities.

Job description

The purpose of this job is to ensure residents receive appropriate personal care and are enabled to attain their optimum level of functioning. To ensure that the center remains in compliance with organization, local, state and federal regulations.

Key Responsibilities:

• Coordinates resident care in conjunction with Administrator, Director of Nursing and other disciplines within Center to ensure quality care. * Ensure care plans are implemented and updated based on resident needs.
• Exercises effective decision making to maximize best clinical outcomes for residents.
• Answer call lights and communicate needs to appropriate person when unable to independently meet the request. Demonstrates ability to prioritize call light request and to make certain all are taken care in a timely manner* Support survey readiness and quality assurance activities.
• Completes daily rounds with incoming shift. Provides shift report to oncoming personnel, which is accurate, concise and pertinent to the resident's care. Coordinates the dissemination of resident information in an appropriate and timely manner

Required:

• Possess a Licensed Practical Nurse License from the State that is in good standing.

Physical & Work Requirements

• Ability to lift 80--300 lbs with assistance/equipment.
• Frequent standing, walking, and responding to emergencies.
