Base pay range

$61,607.00/yr - $75,740.00/yr

Title: Nurse (LPN or RN)

Location: Bronzeville, traveling to other programs in Beverly and Rogers Park

Schedule: On-site, Monday to Friday (8:00am-4:30pm)

Summary:

The Nurse is primarily responsible for supporting the medical team, providing basic medical care, vaccinations, assisting with various patient care activities, and performing related administrative and clerical functions.The Nurse is responsible for respecting and demonstrating patient confidentiality, being culturally competent, and working within the established parameters of the job at all times. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Duties & Responsibilities

  • Administers medications and administers immunizations intramuscularly, intradermally or subcutaneously under the direction of a nurse or physician.
  • Administers point of care tests, such as pregnancy tests.
  • Educates participants on immunizations, medications, preventative care and general health.
  • Educates non-medical staff on medications and serves as resource to staff to receive questions and answer as appropriate.
  • Observes participants post-treatment, charts and reports changes in conditions, such as adverse reaction to medication or treatment, and takes any necessary action.
  • Takes and records patient history and complaints, temperature, pulse, respiration, blood pressure, height and weight, reviews medications with patient and documents med sheet.
  • Keeps charts current with test results and other medical records, including documenting medication, dose and site of injection, among other items.
  • Assists physicians and nurses with procedures in accordance with established protocols.
  • Assists in implementing and maintaining protocols for infectious diseases, such as chicken pox and scabies in a residential setting.
  • Participates in health screenings, educational programs, and team meetings.
  • Prepares participants for their visit with the nurse or physician, examination procedure and/or treatment in accordance with established procedures.
  • Maintains patient flow by escorting participants to the examination room.
  • Under direction of a physician or nurse, provides assistance with placing telephone calls to pharmacies to request prescription refills, as specifically ordered by a physician.
  • Pulls, processes and files medical records.
  • Works in tandem with medical and program staff to ensure participant’s medical records are properly relayed to and from external providers. Assists as a liaison between medical team and other personnel.
  • Maintains inventory of office and medical supplies.
  • Ensures compliance with the Vaccines for Children (VFC) Program.
  • Maintains a high-quality patient care environment by cleaning, preparing, and stocking all examination rooms and treatment areas in accordance with established standards.
  • Assists in the care, cleaning, sterilization, and maintenance of medical equipment and instruments.This includes the monitoring of relevant expiration dates, established maintenance schedules, and assistance in maintaining medical supply inventories.
  • Understands, respects, and demonstrates patient confidentially in all endeavors.
  • Transports participants and supplies as needed, which may include driving an agency vehicle.
  • Works in a general capacity in the milieu as needed.
  • Monitors participants in common areas, such as cafeterias and hallways, as needed.
  • Initiates requests for assistance or guidance from supervisor to address new issues or complex concepts affecting adherence to professional standards.
  • Adheres to professional standards as outlined by governmental bodies, American Nurses Associations (ANA), Nurses Association of College of Obstetrics and Gynecology (NAACOG), the American College of Nurse Midwives (ACNM), private funding sources, Agency plans/policies and unit guidelines.
  • Participates in periodic evaluative reviews and/or in house and external staff training to ensure that he/she understands and continues to adhere to such standards. Initiates requests for assistance or guidance from Supervisor to address new issues or complex concepts affecting adherence to professional standards.
  • Other duties may be assigned.

Education And Experience

  • Graduate of a Licensed Practical Nursing program (LPN) OR graduate of a Bachelor of Science in Nursing (BSN) program.
  • One year of experience in nursing, case work, human services or related field is required.
  • Experience with children and adolescents is preferred.

Certifications/Licenses

  • Active Illinois LPN or RNlicense in good standing.

Knowledge/Skills/Abilities (K/S/A)

  • Bilingual ability is preferred, Spanish is a preferred language.
  • Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to write speeches and articles using original or innovative techniques or style. Ability to make effective and persuasive speeches and presentations on controversial or complex topics to top management, public groups, and/or boards of directors.
  • Access to regular, dependable and timely transportation
  • Ability and comfortability working with people of varying socioeconomic backgrounds.
  • Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders.
  • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner.
  • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Human Care Services.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work independently and remain self-motivated with minimal direct supervision.
  • Strategic thought partnership and consultancy skills.

Work Arrangement

This role is required to work on-site and may require travel to more than one site during a day. In-person meetings and trainings may be required on a regular basis (weekly, monthly, etc).

  • Each program site operates on a 24/7 basis, 365 days per year. To meet both the needs of our participants and contractual obligations, employees are required to be flexible in regard to scheduling and work location.
  • Program sites are located throughout the Chicago area, including the North and South sides of Chicago, as well as the suburbs. Employees may be required to report to any one of our locations on a temporary or permanent basis.
