Job description
Position Overview:
We are seeking a Full-Time dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will collaborate with other medical professionals to provide excellent patient care in our
ICFIID Program. The successful candidate must demonstrate exceptional clinical skills, strong attention to detail, and possess a caring and empathetic nature. This role requires a high level of professionalism, effective communication, and the ability to multitask efficiently.
Responsibilities:
- Performs nursing assessment, prepares nursing care plans and nursing reports
- Participates in ID team meetings
- Consults with physicians, consultants and other clinical and medical professionals regarding individual psychological and medical plans and treatments under the guidance of the Chief Community Living Officer and clinical supervision of RN Serves as liaison with medical consultant and primary physicians
- Manages orders for medications, medication storage, medical supplies and equipment for individuals within the ICFIID program
- Transports individuals to medical and psychological appointments when needed
- Develops and implements medical protocols and tracking charts with RN
- Assists with the planning and organizing of staff orientation and in-service training for Direct Support Professionals and other agency staff
- Administers individual medications when needed
- Maintains valid Virginia driver's license and insurable under VersAbility Resources, Inc., auto insurance
- Participates in an on-call schedule
- Provide support for community integration efforts and HCBS Rights Compliance
- Performs other nursing related duties as needed
Qualifications:
- Current LPN license in the state of Virginia is required.
- Two years of nursing experience in mental health or related field.
- Clinical experience in a healthcare setting preferred.
- Knowledge of nursing principles and practices, including infection control and medication administration.
- Proficient in using electronic health record systems and other healthcare software applications.
- Excellent interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and healthcare team members.
- Strong critical thinking abilities and abilities to make quick and sound decisions in emergency situations.
- Physical stamina and ability to lift, push, and pull heavy objects and provide assistance to patients as needed.
