Job summary

DeKalb CSB Winn Way Outpatient seeks a Registered Nurse or Licensed Practical Nurse to deliver nursing care in an outpatient behavioral health setting. You will assess, plan, and educate clients and families, including venipuncture and injections.

Clinic hours are 8:15am-5:00pm with occasional evenings or weekends. Coordinate care with physicians and other providers; lead in emergencies as needed. Georgia licensure required.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed to practice in Georgia.
• Must be able to perform venipuncture and administer injections.
• Experience with behavioral health is preferred.

Responsibilities

• Perform nursing assessments of clients of all ages and determine interventions based on assessments.
• Document results of assessments and formulate nursing interventions.
• Perform venipuncture and CLIA-waived testing (urine drug screen, breathalyzer).
• Administer injections as ordered by physician/APRN.
• Complete AIMS assessments as ordered.
• Respond to physician/APRN orders promptly.
• Coordinate medical and psychiatric care with community providers and follow up.
• Provide health-related training/education to clients and families as indicated.
• Provide clinical supervision for assigned nursing staff and ensure safe care.

JOB SUMMARY

Under general supervision provides professional nursing services for individuals receiving behavioral healthcare in accordance with medical protocols. Assesses the client's condition, develops care plans, and provides treatment and education. Participates as a member of the clinical treatment team and offers guidance on matters related to nursing treatment and health related matters. Provides direct nursing services and/or clinical education services to clients and their families/significant others.

PROVIDES DIRECT NURSING CARE SERVICES AND/OR CLINICAL EDUCATION TO CLIENTS AND THEIR FAMILIES/SIGNIFICANT OTHERS.

• Performs nursing assessments of clients of all ages to determine current status of client and acts to perform needed interventions based on results of that assessment.
• Documents results of assessments and formulates nursing interventions according to assessment results.
• Performs venipuncture and other CLIA-waived testing, including urine drug screening and breathalyzer, etc.
• Administers injections.
• Completes AIMS Assessments as ordered by the MD or APRN.
• Ensures physician/APRN orders are responded to in a timely manner.
• Facilitates collection of laboratory specimens to include ensuring proper completion of the receiving facilities' requisition form
• Demonstrates knowledge of normal and abnormal lab values.
• Measures and records client vital signs including height, weight, temperature, blood pressure, pulse and respiration.
• Coordinates medical and psychiatric care with community providers ensuring appropriate referral documentation and follow-up from consultation.
• In absence of the physician or APRN, assumes leadership role in medical emergency, providing direction to other team members and making decisions regarding additional emergency care as dictated by agency policies and procedures.
• Provides health related training / education for clients and families as indicated by assessments or at the treatment team request. Facilitates health education groups as needed.

PROVIDES CLINICAL SUPERVISION FOR ASSIGNED NURSING SERVICES STAFF.

• May provide clinical supervision for Licensed Professional Nursing staff, as assigned.
• Contributes professional nursing knowledge in client care decision making
• Delegates or follows through with assignments to promote safe and optimal care of the client and the other staff.

ENSURES OPTIMAL LEVELS OF COMMUNICATION

• Exhibits clear and professional communication with clients, and other staff at all times.
• Communicates information clearly to physicians and to other providers to ensure optimal client care, including any adverse reactions to medications.
• Effectively communicates to families, other Claratel programs, and/or community partners (hospitals, clinics, other MH or SA providers) as needed to promote client care, staff functioning, or business processes.
• Completes all documentation within 24 hours of service delivery.

SKILL, COMPETENCY AND EDUCATION AND/OR LICENSURE REQUIREMENTS

Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed to practice in the State of Georgia. Must be able to perform venipuncture and medication injections. Must possess basic computer and typing skills. Ability to use an electronic health record. Prior experience working with behavioral health preferred.

INTER-RELATIONSHIPS

Able to professionally interface with clients, families, and caregivers with a recovery-focused and trauma-informed approach. Able to communicate effectively with co-workers at multiple sites, internal and external customers and the public.

WORKING CONDITIONS

Works in an outpatient clinic 8:15am-5:00pm with some evening hours and/or weekend hours (as determined by clinic needs). Flexibility to float to other agency sites as needed to assure coverage.

NEEDED ATTRIBUTES

Must be well organized and client focused with excellent interpersonal and customer service skills. Must be able to provide direction to other staff in emergency situations (behavioral and medical). Must be able to work in a recovery oriented, client focused, and trauma informed care environment.
