Base pay range

$35.00/hr - $50.00/hr

This is a full-time remote role for a Nurse (L.V.N or R.N.) RPM & CCM Program at ProMedix Health.

Role Description

Title: Remote Nurse (L.V.N or R.N) for RPM & CCM Programs

Location: Fully Remote (Must be available from 6:00 AM to 6:00 PM Pacific Time)

Licensing Requirement:

• Must hold an active LVN or RN license in the state of California.
• Candidates with a Compact Multi-State License are highly preferred.
• Bilingual proficiency (English/Spanish) is a strong advantage.

Responsibilities

• Deliver exceptional care to patients enrolled in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
• Use RPM technology to monitor patients’ health metrics and provide timely interventions as needed.
• Coordinate care management programs for individuals with chronic conditions, ensuring alignment with treatment goals.
• Conduct clinical assessments and develop tailored care plans for optimal patient outcomes.
• Provide patient education and support, empowering them to manage their health effectively.
• Collaborate with healthcare providers and clinical teams to ensure seamless care delivery within the RPM and CCM frameworks.
• Maintain accurate and thorough documentation in compliance with HIPAA, using Electronic Medical Records (EMR) systems.
• Stay informed and compliant with California and federal nursing regulations.

Schedule and Compliance

• Compensation: Based on experience and qualifications. Competitive hourly rates will be discussed during the interview process.
• Must be available for shifts from 6:00 AM to 6:00 PM Pacific Time (flexible scheduling within this timeframe).
• Employment is contingent upon the successful completion of a comprehensive background check.
• Strict adherence to the California Nurse Practice Act and all federal and state regulations for nursing practice.
• Compliance with ongoing continuing education requirements for LVNs and RNs in California.
