A healthcare company is seeking a Telephonic Nurse Disease Management II to provide patient education and health coaching for chronic diseases. This role allows for virtual work with required in-person training. Candidates should have an AS in nursing and a current RN license. Preferred skills include case management experience and knowledge of Coronary Artery Disease and COPD.
Job Title: Telephonic Nurse Disease Management II
Location: This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy.
Work Schedule: Monday-Friday, 12:30 - 9 pm EST.
The Telephonic Nurse Disease Management II is responsible for participating in and delivery of more complex patient education and disease management interventions and performs health coaching for members, across multiple lines, for health improvement/management programs for chronic diseases.
