Ottawa County Community Mental Health is seeking a Mental Health Nurse (DD Services) to deliver health assessments, teaching, counseling, direct care, and case management for consumers in the Holland area. This full-time position operates 8:00 a.m. to 5:00 p.m.

Monday through Friday, with benefits provided. The role involves collaborating with a range of health partners, maintaining documentation, and ensuring compliance with state and federal regulations.

Qualifikationen

• BSN from an accredited college or university or RN with 3+ years of experience in developmental disabilities.
• Licensed to practice in Michigan; valid driver's license required.
• Equivalent education/experience acceptable.

Aufgaben

• Provide nursing services and case management to developmentally disabled individuals in homes and community settings.
• Conduct health assessments, counseling, teaching, and referrals.
• Coordinate care with physicians, caregivers, and supports coordinators.
• Maintain documentation and ensure HIPAA compliance and privacy.
• Develop and monitor individualized nursing care plans and follow-up services.
