Job summary

Neighborhood Health is seeking a Nursing Coordinator in Hybla Valley, VA to support access to high-quality, patient-centered primary care. You will supervise the clinic's clinical staff, oversee triage and high-risk case management, and collaborate with the Nurse Supervisor and Director of Nursing to ensure safe care for all patients.

The role requires Virginia RN licensure with possible compact state recognition and experience in primary care settings.

Qualifications

• Registered Nurse licensed in Virginia or compact state.
• BSN preferred.
• Experience in a primary care environment, preferably with Federally Qualified Health Center.
• Ability to work with diverse and low-income patient populations.
• Knowledge of local health care environment and capacity to develop partnerships with community partners.

Responsibilities

• Coordinate patient-centered primary care with multidisciplinary team.
• Supervise clinical site staff and manage schedules and training.
• Facilitate medical flow and ensure safety and quality standards.
• Oversee supplies, medications, and equipment and obtain prior authorizations.
• Promote Primary Care Medical Home practices and vaccine programs.
• Manage high-risk patient care and chronic disease management.
