Job summary

National Care Advisors LLC seeks an experienced Field Nurse Case Manager to deliver regional case management for elder care. You will work with families, attorneys and financial planners to ensure quality of life and care planning across multiple sites in the western U.S.

The role requires BSN and 5+ years nursing experience, with CCM/CRRN preferred. Remote-friendly, with regional travel 1-2 times monthly and occasional nationwide travel.

Qualifications

• BSN required; CCM or CRRN preferred.
• Minimum of 5 years clinical nursing experience, with field case management or workers’ compensation case management preferred.
• Experience with third-party benefits – health insurance, Medicaid, Social Security, Developmental Disability services.
• Excellent communication skills – written and verbal.
• Highly motivated self-starter comfortable working in a virtual company/office, with solid time management and organization skills.
• Proficient in MacOS and a variety of related software applications (including MS Office).
• Strong ability to quickly learn and adapt to new technologies and tools in a Mac environment.
• Proven ability to collaborate as necessary to accomplish goals and work through conflicts.

Responsibilities

• Provide regional case management services and elder client care considerations.

• Overnight travel regionally 1-2 times per month and occasional nationwide travel.

POSITION OVERVIEW

This position will be responsible for regional case management services responsive to special needs and elder client care and quality of life challenges. This position requires collaboration with the primary family caregivers, attorneys, financial planners and trustees that are also serving the client.

Requirements
