Southern Arkansas University is seeking a Nurse (Consultant) to collaborate with physicians, social workers, and other care professionals in Fredericksburg, VA, to coordinate patient-centered care and support clinical outcomes.

The role involves working within a nurse and social work case management team, contributing to care planning, patient education, and ongoing assessments to ensure continuity of care and high-quality results.

Responsabilidades

• Collaborate closely with physicians, advanced practice providers, social workers, therapists, laboratory personnel, and other disciplines to coordinate patient-centered care.
• Function as part of a nurse and social work case management team, working jointly to address medical, psychosocial, and supportive care needs.
• Contribute to individualized care planning, patient education, and ongoing clinical assessment to support continuity of care and optimal patient outcomes.
• Provide direct patient care, including management of multidrug therapy, monitoring for adverse drug reactions, wound care, and prevention of disability.
