General Summary Of Position

Primary Duties And Responsibilities

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Performs general nursing care to patients. Administers prescribed medications, vaccines, treatments, and procedures in accordance with nursing standards. Observes, records, and reports patient's condition and reaction to drugs and treatments to physicians. Dispenses medication as directed. Ensures that nursing documentation is complete, accurate, and legible. Greets patients and prepares them for physician examination. Screens patients for appropriate information. Instructs and teaches patients in the collection of samples and tests. Prepares equipment and aids physician during treatment, examination, and testing of patients.
  • Develops weekly schedules and makes daily assignments. Serves as the point of contact for all clinic personnel changing shifts or calling in regarding attendance matters.
  • Participates with the Administrator I in quality and process improvement initiatives and assists in maintaining fiscal objectives. Performs mock The Joint Commission surveys to ensure regulatory compliance.
  • Performs patient telephone triage, assesses needs, and coordinates needs with nursing and medical staff. Also provides coverage to nurses within the unit.
  • Educates/counsels patient and family about diagnostic procedures, medication adherence, nutrition, and maintenance of health and wellness. Maintains and reviews patient records, charts, and other pertinent information. Posts tests and examination results. Educates patients by providing information concerning diagnosis related medications, side effects, and the disease process.
  • Instructs patients and family regarding medications and treatment/surgical instructions. Provide patient education and serves as a resource to other clinical team members regarding particular expertise. Instructs patients with postoperative instructions following routine surgery prior to postoperative follow-up care. Contributes to the implementation of clinic objectives and quality assurance programs.
  • Orders and evaluates appropriate follow-up diagnostic studies and interprets and synthesizes data to determine therapeutic plan.
  • Maintains established departmental policies and procedures; attends required meetings and participates in committees as requested. In conjunction with the Nurse Manager develops and supports continuing education opportunities for clinic staff. Maintains own clinical expertise by attending in service programs.
  • Works with Nurse Manager to ensure staff are trained and adhere to office policies and procedures, including adherence to minimum service requirement standards.
  • Adapts to and excels in the use of all electronic programs utilized in the division to include, but not limited to, Centricity, Azyxxi and McKesson. Documents all vital signs, treatments and medication immunization administration promptly.
  • Monitors patient progress and referrals through regularly scheduled face to face appointments, telephone calls, and hospital visits. Coaches patients receiving antiretroviral therapy towards successful treatment outcomes.
  • Screens for basic entitlement eligibility (ADAP, MADAP, MHIP, PAP) and complete applications or referrals as appropriate.
  • Manages, monitors and exercise all functions related to the Division's prescription refill line.
  • Collects clinical data relevant to initiate and complete prior authorization for oral medication.
  • Participates in weekly case conferences and work within a multidisciplinary team on assessment and treatment planning.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
  • Participates in multi-disciplinary quality and service improvement teams.
  • Essential member of care team. Provides patient care for the Pulmonology and Neurology Division under the supervision of the attending physician. Reviews treatment plans and provides patients and families with education and instruction for care. Documents individualized nursing care plans for patients. Provides case management with approved treatment plans as well as information and instruction regarding diagnosis, treatments, options, side effects, and diagnosis. Assists MDs in the performance of Pediatric Pulmonology and Neurology procedures as requested.
  • Provides patient care via telephone for patients under the care of the Pediatric Pulmonology and Neurology Division. Triages calls and coordinates with medical staff and/or other resource to ensure appropriate care. Returns phone calls within 24 hours they are received, obtains accurate information regarding patient symptoms and current medication regime. Refills prescriptions under attending physician's supervision and work on prior authorizations for medications when needed. Documents phone calls and notifies attending physician of pertinent information.
  • Collaborates in the development of principles of patient care while maintaining communication with appropriate allied services and agencies. Promptly requests consults from other services when indicated. Sends referrals to appropriate agencies and/or support systems to facilitate patient well-being. Works with department administration and CYAS Nurse Manager to facilitate and improve patient care. Follows up with Pulmonology and Neurology patients who have recently undergone procedures or surgery, under the supervision of attending physicians, to ensure appropriate follow-up, educational materials, discharge plans, etc. Screens clinic patient per protocol.

Minimum Qualifications

Education

  • Associate's degree Graduate of an accredited School of Nursing. required and
  • Bachelor's degree In Nursing (BSN) preferred and
  • One year of relevant education may be substituted for one year of required work experience.

Experience

  • 1-2 years 2 years of professional nursing experience; must have strong organizational skills. required and
  • 1-2 years Previous office management/administrative experience . 1 year of previous outpatient experience preferred
  • One year of relevant professional-level work experience may be substituted for one year of required education.

Licenses and Certifications

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Registered Nurse, licensed in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s) Upon Hire required and
  • CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required and
  • Additional unit/specialty certifications may vary by department or business unit.

Knowledge, Skills, And Abilities

  • Knowledge of professional nursing theory and practice to assess, plan, implement, and evaluate patient care.
  • Knowledge of medical equipment and instruments to administer patient care;
  • Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care;
  • Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public.
  • Ability to react calmly and effectively in emergency situations.

This position has a hiring range of $34.98 - $62.52
