Job description

Join to apply for the Nurse Case Manager - San Francisco role at Paradigm.

We are seeking a Field Nurse Case Manager to cover San Francisco, Daly City, CA, and surrounding areas. This is a home-based position with travel, up to 2 hours one-way, to doctors’ offices, hospitals, and other locations. The role involves medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager collaborates with insurance carriers, medical providers, attorneys, employers, and employees, monitoring progress and reporting results.

Qualifications include:

  • Current, unencumbered RN license in California
  • Experience in workers' compensation case management preferred
  • Certifications (CCM, CRRN, CDMS, CRC) are strongly preferred; must be obtained within two years of hire
  • Skills: organization, technology proficiency, communication, autonomous operation, patient education, valid driver’s license and good driving history
