Incredible Health is hiring a Nurse Case Manager (RN) in the Selma, CA area. The role requires an active RN license, BSN or higher, and 1+ years in case management within a hospital setting.

Collaboration with clinicians and families is essential to design and monitor individualized care plans. You will coordinate care, document progress, and ensure privacy compliance while serving as the primary liaison across the healthcare team.

Qualifications

• BSN or higher from an accredited program
• Active and unencumbered RN license

• Strong interpersonal and communication skills to collaborate with providers, patients and families
• Proficient in electronic health records (EHR) software

Responsibilities

• Conduct comprehensive patient assessments including medical history, medications, and psychosocial needs
• Collaborate with healthcare providers, patients, and families to develop individualized care plans
• Monitor and evaluate patient progress, adjusting plans as needed and communicating changes to the care team
• Serve as main contact between patient and healthcare providers to ensure seamless communication
• Maintain up-to-date records and case notes in compliance with confidentiality standards

Qualifications

• Bachelor of Science in Nursing (BSN) or higher educational attainment from an accredited program
• Active and unencumbered Registered Nurse license
• 1+ years experience in case management, preferably within a healthcare or hospital setting
• Exceptional interpersonal and communication skills, both written and verbal, to effectively collaborate with medical professionals, patients, and family members
• Proficiency in electronic health records (EHR) software
