Incredible Health is actively hiring a Nurse Case Manager (RN) in the Mission Hills, CA area. The role covers comprehensive assessments, care plan development with healthcare teams, and ongoing patient progress monitoring.

Flexible shifts include day, night, and mid shifts with options for full-time, part-time, or per diem work. Qualifications include a BSN or higher, active RN license, 1+ year case management experience, excellent communication, and proficiency with EHR software.

Responsibilities

• Conduct comprehensive assessments of patients and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement care plans.
• Monitor and evaluate patient progress and adjust care plans as needed.
• Serve as main point of contact between patient and healthcare providers to ensure seamless coordination.
• Maintain up-to-date records and case notes with confidentiality and compliance.

Qualifications

• Bachelor of Science in Nursing (BSN) or higher educational attainment from an accredited program
• Active and unencumbered Registered Nurse license
• 1+ years experience in case management, preferably within a healthcare or hospital setting
• Exceptional interpersonal and communication skills, both written and verbal, to effectively collaborate with medical professionals, patients, and family members
• Proficiency in electronic health records (EHR) software
