Job summary

Incredible Health is seeking a Nurse Case Manager (RN) in Fresno, CA, to coordinate patient care and collaborate with healthcare teams. The role requires a BSN or higher and an active RN license, with 1+ year of case management experience.

You will conduct assessments, develop individualized plans, monitor progress, and maintain confidential records in an integrated hospital setting. Eligible shifts include day and night, with both full-time and part-time options.

Qualifications

• 1+ year experience in case management in healthcare.
• Excellent written and verbal communication; able to collaborate with providers, patients, and families.

Responsibilities

• Conduct comprehensive patient assessments (medical history, medications, treatment plans, psychosocial needs).
• Collaborate with healthcare providers, patients and families to develop and implement care plans.
• Monitor and evaluate patient progress; adjust care plans as needed and report changes to the team.
• Serve as main contact between patient and providers to ensure seamless communication and coordination.
• Maintain accurate case notes, ensuring confidentiality and compliance.

Qualifications

• Bachelor of Science in Nursing (BSN) or higher educational attainment from an accredited program
• Active and unencumbered Registered Nurse license
• 1+ years experience in case management, preferably within a healthcare or hospital setting
• Exceptional interpersonal and communication skills, both written and verbal, to effectively collaborate with medical professionals, patients, and family members
• Proficiency in electronic health records (EHR) software

Responsibilities
