Job summary

American Senior Communities is seeking a Registered Nurse case manager to join our interdisciplinary team. The role supports resident care planning for a designated unit, in collaboration with the Unit Manager and ADNS, and adheres to federal/state regulations and facility policies.

Ideal hours 10am-6pm with on-call every 7th weekend. Responsibilities include assessments, documentation of changes, and maintaining compassionate geriatric care.

Qualifications

• Current and valid Registered Nurse license in Indiana.
• Proficient medication management skills.
• Ability to conduct thorough assessments and accurately document changes in resident conditions.
• Strong passion for geriatric nursing and commitment to senior care excellence.
• Demonstrates CARE values to residents, families and staff.

Responsibilities

• Plan, assume responsibility and accountability for resident care on a designated unit with the Unit Manager and ADNS.
• Coordinate care with nursing and facility teams to ensure comprehensive resident care.
• Ensure compliance with Federal/State regulations and facility policies and care plans.
• Contribute to an interdisciplinary team to optimize resident outcomes.

The Case Manager serves as a member of the interdisciplinary team, assumes planning, responsibility, and accountability for resident care of a designated unit, in conjunction with the Unit Manager (UM) and or Assistant Director of Nursing Services (ADNS), and in accordance with Federal and State regulations and company/facility policies, procedures and care plans.
