Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• RN licensure required; 5 years recent clinical experience preferred.
• Knowledge of clinical practice guidelines, databases, and payer protocols desired.

Responsabilités

• Coordinate patient care from pre-admission through post-discharge.
• Collaborate with physicians and nurses to manage transitions of care.
• Utilize nursing processes to monitor and optimize each patient’s plan of care.
