Job summary

MDH Dorchester County Health Department is seeking a full-time Nurse with a focus on Mobile Integrated Health to provide resource navigation and linkage to health service connections in Cambridge, MD. The role includes community-based follow-up and conducting health education during home visits.

Applicants must hold a Maryland RN license and a driver’s license, with substitutions allowed for related field degrees and RN experience.

Qualifications

• Bachelor’s degree in nursing from an accredited college or university.
• Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a Registered Nurse for the required education.
• Must possess a current license as a Registered Nurse from the Maryland State Board of Nursing.

Responsibilities

• Provide resource navigation and linkage to health service connections for the Mobile Integrated Health program.
• Facilitate community-based follow-up for the Mobile Integrated Health Program.
• Participate in home visits where health education occurs.
• Engage in community outreach and act as a community liaison for health planning committees.
