Job summary

State of Maryland is seeking a nurse to serve as the primary nursing professional for the Kent County Health Department's Mobile Integrated Community Health (MICH) Program. The role involves community-based assessments, care coordination, disease management, and education for adults who frequently use EMS.

You will collaborate with EMS, pharmacists, and social services, maintain a caseload, and participate in program evaluation.

Qualifications

• Bachelor’s degree in nursing from an accredited college or university is required.
• Current RN license in Maryland is required.
• Substitutions for education or experience are allowed as specified in notes.

Responsibilities

• Conduct in-home and community nursing assessments and health histories.
• Develop individualized care plans and provide health education.
• Coordinate referrals and monitor client progress to reduce avoidable EMS usage.
• Collaborate with EMS, EMTs, pharmacists, and other partners in care planning.
• Maintain an assigned MICH caseload and participate in program evaluation.
