Job summary
Tustin Healthcare Center, a Generations Healthcare facility, is hiring a Nurse Case Manager to develop, evaluate, and implement care plans for residents at our skilled nursing and rehabilitation center in Tustin, CA.
The role requires an active RN license in California, supervisory experience in healthcare, and strong communication and organizational skills. Competitive compensation and benefits accompany a commitment to quality patient care and teamwork.
Qualifications
- Must possess a Nursing Degree from an accredited college or university.
- Must have at least one (1) year of supervisory experience in a hospital, nursing care facility, or other related health care facility.
- Must have training in rehabilitative and restorative nursing practices.
- Strong communication, excellent customer service, teamwork and organizational skills.
- Must possess a current, unencumbered, active RN license in this state.
- Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
- Be a supportive team member and contribute to team work and team concept.
Responsibilities
- Evaluate and update patient health care plans.
- Assessment for new patients.
- Review test results from medical exams.
- Implement Physician orders and follow up with treatment plans.
- Communicate with families, health providers and patients.
- Documentation of medical records.
- Provide emotional support to families and patients.
- Provide education about health care plans to patients and families.
