Generations Healthcare's Tustin Healthcare Center seeks a Nurse Case Manager (LVN) to join our team in Tustin, CA. The role focuses on developing and evaluating health care plans, managing cost-effective medical plans, and coordinating care with families, doctors, providers, and patients.

Ideal candidates possess a nursing degree, at least one year of supervisory experience in a hospital or care facility, and an active nursing license. The position offers $75-$80K DOE and full-time hours.

Qualifications

• Must have Nursing Degree from an accredited college or university.
• Minimum 1 year of supervisory experience in a hospital, nursing care facility, or related health care facility.
• Training in rehabilitative and restorative nursing practices.
• Must possess a current, unencumbered, active license to practice as an RN in this state.
• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.

Responsibilities

• Evaluate and update patient health care plans.
• Assess new patients.
• Review test results from medical exams.
• Implement physician orders and follow up with treatment plans.
• Communicate with families, health providers and patients.
• Document medical records.
• Provide emotional support to families and patients.
• Educate patients and families about health care plans.
