Job summary

University Health System- San Antonio seeks a registered nurse to coordinate the interdisciplinary treatment plan of care for patients across the healthcare continuum. The role includes facilitating service delivery, evaluating outcomes, and advocating for patient health care needs.

The ideal candidate has a nursing degree (BSN/MSN preferred) with two years of acute care experience (ICU preferred) and case management exposure, plus certification in case management is desirable.

Qualifications

• Graduation from an accredited school of professional nursing is required.
• Bachelor's degree or Master's degree in Nursing is highly preferred.
• Two years of acute nursing experience, preferably in Intensive Care, is required.
• Two years experience in any case management care setting; HMO, Physician Group Model, MSO, or Acute Care is required.
• Five years nursing experience preferred (Staff nurse II or above).
• National certification in case management is preferred.

Responsibilities

• Coordinates the interdisciplinary treatment plan of care for patients across the healthcare continuum within a defined population.
• Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs.
