Qualifications

• 5 years Clinical Experience in Case Management, Utilization, Discharge Planning.
• Minimum of 2 years of leadership or managerial experience in a healthcare environment.

Responsibilities

• Direct, supervise, and manage the activities of the Case Management Team.
• Act as a resource person for case management team and discharge planning.
• Coordinate patient care with all hospital departments, community agencies, and extended care facilities.
• Provide verbal and written explanation of patient's plan of care and discharge plan.

Education

BSN Required. MSN preferred

Current and valid NYS licensure required
