Nash UNC Health Care in Nash County, NC is seeking a Nurse Case Manager to assist the Director in organizing departmental activities and evaluating the case management process. Qualifications

• Graduate of an approved nursing program.
• Current License to practice as a Registered Nurse in North Carolina.
• BCLS Certification.
• ACLS Certification.

Responsibilities

• Assists the Director in organizing departmental activities, evaluating case management process, evaluating personnel, and implementing the Orientation and Competency Program.
• Coordinates & provides case management to an assigned unit based on staffing needs at least 50% of scheduled time.
• Serves as clinical resource for the patient, staff, and other clinical departments.
• Assists nursing staff with priority setting.
• Demonstrates clinical expertise in execution of the nursing process for case management.
• Collaborates with the Care nursing and other disciplines regarding patient and family education and discharge planning needs.
• Monitors hospital length of stay and collaborates with medical staff for discharge planning.
