You will assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize health care across the care continuum and ensure access to services aligned with patient needs. Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Requires a Massachusetts license to practice professional nursing.
• Minimum 1–3 years of experience in addiction medicine and/or behavioral health.
• Knowledge of nursing and medical principles and ability to apply them to practice and procedures.

Responsibilities

• Interviews patients for complete medical histories and designs a nursing care plan in collaboration with the patient.
• Assesses and monitors patients during induction, stabilization, and maintenance phases of treatment.
• Monitors follow-up visits, medication review, behavioral health compliance, and relapse prevention.
• Obtains prior authorization for Suboxone and/or Vivitrol and coordinates with pharmacy for orders and refills.
• Documents patient needs clearly and communicates plans through symptoms and assessments; provides patient education.
