Qualifications

• Must hold an active RN license in the applicable state.
• Associate degree in nursing is required; BSN preferred.
• Minimum of five years in clinical roles including utilization or case management.
• Certification in case management or utilization management preferred.

Responsibilities

• Assess discharge needs for inpatient and ED patients and plan post-acute services.
• Collaborate with providers, social work, and the care team to develop patient-centered plans.
• Monitor progression, barriers to discharge, and resource utilization; escalate as needed.
• Educate patients and families on discharge processes and medications.
• Coordinate referrals to home health, DME, transportation, and other services.
• Ensure timely delivery of discharge resources and transitions of care.
