Job summary

Paradigm is seeking a full-time Field Nurse Case Manager to cover the St. Louis, MO area. This is a 0.75–1.0 FTE role with base salary ranges and travel involved.

The RN must manage work-related injuries, coordinating care with insurers, providers, and employers, and maintain strict timelines. A valid driver's license is required; certifications are encouraged and may be obtained within two years.

Qualifications

• Current, unencumbered compact RN license (KS or KS+IL) required.
• Experience in workers' comp case management preferred.
• Certifications CCM/CRRN/CDMS/CRC recommended; required within two years of hire.
• Strong organization, time management, PC/technology and communication skills.
• Ability to operate autonomously with minimal oversight.
• Patient education skills.
• Valid Driver’s License with good driving history.

Responsibilities

• Provide medical case management for work-related injuries.
• Assess, plan, coordinate, implement and evaluate patient care.
• Collaborate with insurance carriers, medical providers, attorneys, employers and employees.
• Monitor progress and report results to employers and carriers.

Paradigm is looking for a full‑time, 0.75 - 1.0 FTE (30-40 hours per week), benefited Field Nurse Case Manager to cover the St. Louis, MO, area. This is a salaried position: base salary range at 0.75 FTE is $68,500 - $72,500. At 1.0 FTE, base salary range is from $84,000 - $92,500 annually.

A Field Case Manager role is a field / home-based position with travel, up to 2 hours one-way (4 hours round trip per appointment). These appointments would be to doctors’ offices, hospitals, and various other locations. There may be multiple appointments in 1 day.

This individual is responsible for the medical case management of work‑related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.

Qualifications:

• Current, unencumbered compact Registered Nurse (RN) license in Kansas or combination of single state Kansas and Illinois RN licenses required
• Experience in case management of workers comp cases, preferred
• Recommended certifications (CCM, CRRN, CDMS, CRC) preferred. REQUIRED within two years of the date of hire
• Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
• Ability to operate autonomously with minimal oversight
• Skilled at patient education
• Valid Driver’s License, and good driving history
