Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the San Diego area in CA and surrounding areas. This home-based role involves travel up to 2 hours one-way to doctors’ offices, hospitals, and other locations.

You will manage medical case work for injured workers, coordinating assessment, planning, implementation, and evaluation while collaborating with insurers, providers, and employers. At Paradigm, people come first.

Qualifications

• Current, unencumbered RN license issued in California.
• Experience in workers' compensation case management preferred.
• CCM certification strongly preferred; must obtain within two years of hire date.
• Strong organization, timeline adherence, communication, autonomy and patient education skills.
• Valid driver’s license and clean driving history.

Responsibilities

• Conduct medical case management for work-related injuries from assessment to evaluation.
• Coordinate care with insurance carriers, medical providers, attorneys, employers and employees.
• Monitor progress and report outcomes to employer and carriers.
• Travel to doctors’ offices, hospitals and other locations as required.

We are seeking a Field Nurse Case Manager to cover the San Diego Area in CA, and surrounding areas.

A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (4 hours round trip per appointment). These appointments would be to doctors’ offices, hospitals, and various other locations. There may be multiple appointments in 1 day. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors progress of the injured worker and reports results back to the employer and insurance carriers.

Qualifications:

• Current, unencumbered Registered Nurse (RN) license in California
• Experience in case management of workers comp cases, preferred
• Certified Case Manager Certification (CCM) strongly preferred; will need to obtain within two years of hire date
• Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
• Ability to operate autonomously with minimal oversight
• Skilled at patient education
• Valid driver’s license, and good driving history
