Job summary

HomeCentris Healthcare, LLC is seeking a PRN Nurse Case Manager in Lanham, MD to provide nursing oversight of nonskilled homecare services and coordinate patient care in clients' homes. This role emphasizes assessment, planning, and documentation to meet each patient’s needs while ensuring compliance with regulations and agency policies.

The RN uses assessment skills to plan and implement home health plans, supervising caregivers and ensuring high-quality patient care in a home setting.

Qualifications

• Certified Delegating Nurse preferred.
• Case management experience in Home Health (1 year) required.

Responsibilities

• Coordinate care to assigned clients, establish a Start of Care and ongoing assessments.
• Perform onsite supervisory visits and document follow-up appropriately.
• Provide guidance, supervision, training and evaluation of caregivers; deliver excellent customer service.
• Maintain professional RN standards of practice.
• Perform other duties as assigned by supervisor.

HomeCentris Healthcare is seeking a PRN RN Case Manager for our Personal Care division in our Lanham office to provide quality care and nursing oversight of its nonskilled homecare services in the setting where our clients most want to be, in their homes. The RN uses assessment skills to plan, coordinate, provide and document care for an assigned caseload of patients. The RN participates in the development and implementation of a home health plan that meets each patient’s specific needs and is in compliance with federal and state regulations and agency policies and procedures.

RESPONSIBILITIES

• Coordinate care to assigned clients, establish a goal-directed care plan/Start of Care, and ongoing assessments of clients' needs.
• Perform onsite supervisory visits to assess client, family, environment, and clinical care givers and complete timely follow-up documentation.
• Provide guidance, supervision, orientation, training, education, and evaluation of caregivers. Provide excellent customer service.
• Maintain a professional demeanor consistent with registered nurse standards of practice.
• Perform other duties as assigned by supervisor.
