Job summary

CP Unlimited is seeking an experienced Nurse Case Manager for our Hudson Valley/Putnam location. You will coordinate health care services for adults with disabilities in residential settings and perform nursing assessments, working with an interdisciplinary team to reach positive outcomes.

Applicants should hold a NYS RN license, have nursing experience with disabilities, and be able to travel across multiple locations. Flexibility in hours is also required for this full-time role.

Our Community Living Services Program is currently seeking full-time Nurse Case Managers for our Hudson Valley/Putnam location.

Nurse Case Manager Responsibilities:

• Coordinate the delivery of health care services for adults with disabilities residing in residential programs;
• Be responsible for performing nursing assessments and diagnoses;
• Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.

Requirements:

• NYS Registered Nurse license. Bachelor’s Degree preferred;
