Job description

Compensation: $34.00/hr

The Nurse Care Manager Trainer/Floater is part of the person-centered integrated care team, providing direct clinical and medical services to persons enrolled in the Health Care Home through a "whole person" approach. This position involves training and onboarding new Nurse Care Managers (NCM) and filling in for NCMs as needed, both virtually and face-to-face.

This is a full-time, benefit-eligible position working Monday-Friday, primarily 8:00 am - 5:00 pm. It is remote but may require minimal travel.

ESSENTIAL FUNCTIONS – JOB SPECIFIC:

  1. Develop onboarding schedules for new DD HH NCMs in collaboration with the Sr. Director of Nursing Services.
  2. Provide onboarding and training to new NCMs, ensuring consistency across the program.
  3. Offer refresher trainings as needed.
  4. Seek feedback on onboarding processes and implement improvements.
  5. Develop and maintain DD HH nursing processes with the Sr. Director of Nursing Services.
  6. Manage access for new employees during onboarding and resignations, including Cyberaccess, HRST, Care Manager portals, Lab Portals, and Contact Profiles.
  7. Serve as a point person for New Employee Orientation alongside the RND.
  8. Provide NCM services as needed, including for those on leave or extra help situations.
  9. Complete monthly interventions and care activities for up to 200 clients, complying with Department of Mental Health regulations.
  10. Conduct health screenings, identify risks, and recommend care during client visits.
  11. Assist in developing physical health goals and promote healthy lifestyles.
  12. Ensure clients are referred to primary care providers and educate them on the importance of PCPs.
  13. Perform metabolic syndrome screenings and medication reconciliation as per policies.
  14. Educate clients and caregivers on health conditions and management.
  15. Collaborate with healthcare providers and human service agencies to ensure holistic care.
  16. Travel to clients’ residences as needed.
  17. Meet service expectations and seek culturally competent services for clients.
  18. Travel for nursing coverage and training as required.
  19. Perform other duties as assigned.

WORKING CONDITIONS & EXPOSURE RISK:

  • Exposure to blood, body fluids, biohazards, and chemicals.
  • Operate vehicles or machinery.

QUALIFICATIONS:

  • High School/GED required; Associate of Nursing degree required; Bachelor’s preferred.
  • At least two years of medical nursing experience preferred.
  • Proficiency in Microsoft Office and Electronic Medical Records preferred.
  • Valid driver’s license and agency-required automobile insurance.

LICENSURE/CERTIFICATION:

Registered Nurse (RN) required.
