As a Nurse Care Manager, you will work closely with internal and external treatment teams to provide education, care coordination, and ongoing support to clients who have or are at risk for chronic medical conditions. Key Responsibilities:
Collaborate with treatment team members to support clients with or at risk for complex medical conditions
Provide individualized health education and training on chronic diseases and medication management
Develop and implement wellness and prevention initiatives
Participate in initial and ongoing treatment planning for Healthcare Home enrollees
Manage hospital and provider admissions/discharges for assigned clients
Facilitate health education groups using advanced knowledge of medical conditions
Consult with treatment teams regarding client health concerns and care planning
Utilize DMH Net Health technology programs including Cyberaccess, Care Manager, and IntellectAbility
Monitor and report performance measures and outcomes using nursing-specific information systems
Participate in quality assurance, clinical documentation, program evaluation, and grant-related initiatives
Access emergency services in crisis situations as needed
Perform other related duties as assigned by supervisor
Education and/or Experience Qualifications:
Active LPN (Licensed Practical Nurse) license required
Experience working with individuals with intellectual and developmental disabilities (IDD) preferred
Required License/Certification:
Current LPN licensure in the state of Missouri
Additional Qualifications:
Strong understanding of chronic disease processes in both children and adults
Excellent written and verbal communication skills
Ability to implement innovative care systems and workflows
Sound decision-making abilities based on clinical reasoning and nursing processes
Ability to work collaboratively in a multi-disciplinary team environment
