Job description

We're looking for Nurse Care Managers for our Care and Case Management team who have experience working with a health plan, health navigator, or third-party administrator (TPA). They should be passionate about caring for members holistically through their healthcare journey and ensuring needs are met with industry-leading interventions. As a telephonic Nurse Care Manager, you will report to the Manager, Care and Case Management, guiding members through complex medical and behavioral health situations. You will partner with a diverse clinical team that includes healthcare professionals, care coordinators, and records specialists to deliver integrated remote care in a creative way.

Responsibilities:

  1. Deliver coordinated, patient-centered virtual Care Management by telephone or video that improves members' health outcomes.
  2. Create impactful care plans with members and our care team, helping members achieve their health goals.
  3. Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system.
  4. Partner with local providers to ensure coordinated care.
  5. Provide compassionate, longitudinal follow-up care and build supportive relationships.
  6. Assist during acute healthcare episodes, such as hospitalizations, providing coordinated Case Management.
  7. Coordinate resources addressing clinical or social issues holistically.

Qualifications:

  1. Bachelor of Science in Nursing (BSN).
  2. Current CCM Certification or eligibility for exam in August 2025.
  3. 5+ years of nursing experience.
  4. 2+ years in care management.
  5. Reside in a compact NLC state.
  6. Active compact RN license in good standing.
  7. Willing to maintain licensure in multiple states.
  8. Work hours from 9AM to 6PM local time.
  9. Experience discussing multiple medical conditions across age ranges.
  10. Remote work experience with proficiency in multiple computer and medical record systems.
  11. Empathetic approach to working with patients and families.
  12. Ability to meet volume goals without sacrificing quality.
  13. Strict adherence to security and HIPAA regulations.
  14. Responsive, collaborative, and eager to learn.
  15. Strong communication skills for clinical history gathering and summarizing findings.
  16. Ability to write clear, patient-centric medical information.

Physical / Cognitive Requirements:

  1. Regular attendance at work location.
  2. Ability to remain seated for prolonged periods.
  3. Manual dexterity for operating equipment.
  4. Occasional exertion of about 20 lbs of force.
  5. Professional interaction with leadership, colleagues, and members.
