Nurse Care Manager (Remote) - Contract Opportunity

Contract Period: 10/28/2026 - 04/02/2027
Location: Remote
Schedule: Monday-Friday, 8:00 AM - 5:00 PM ET
Hours: 40 hours/week
Employment Type: Full-Time Contract

Position Overview

We are seeking experienced Registered Nurses for a remote Nurse Care Manager opportunity. This role is responsible for collaborating with healthcare providers and members to improve health outcomes through personalized care management. The Nurse Care Manager will support individuals with complex and chronic healthcare needs while ensuring quality, cost-effective care.

Responsibilities

• Conduct clinical assessments and develop individualized care plans.
• Manage members with chronic and complex healthcare conditions.
• Review medical necessity for inpatient admissions, continued stays, outpatient services, and out-of-network care.
• Collaborate with physicians, medical directors, and healthcare providers regarding treatment planning and care decisions.
• Coordinate discharge planning and transitions of care.
• Monitor member progress and adjust care plans as needed.
• Ensure compliance with care management standards and clinical guidelines.

Required Qualifications

• Active, unrestricted RN license.
• Minimum of 3 years of acute care clinical experience.
• Strong computer and documentation skills.
• Ability to work independently in a remote environment.

Preferred Qualifications

• Case management experience.
• Managed care experience.
• Telephonic nursing experience.
• Outpatient clinical experience.
• Home health and/or discharge planning experience.
• Associate's or Bachelor's degree in Nursing.
• Case Manager Certification (CCM).

Licensure Requirements

Candidates must hold an active RN license in one of the following states:

• Connecticut (Compact State)
• Missouri (Compact State)
• Nevada (Single-State License)

Ideal Candidate

• Experienced in telephonic case management.
• Background in managed care or outpatient settings.
• Comfortable navigating multiple computer systems.
• Reliable attendance with limited planned time off during the assignment.

Additional Information

• This is a fully remote position; however, candidates must be able to attend onboarding, training, or other required activities as directed.
• This is not a patient-facing role.
• No work will be performed in care centers, clinics, assisted living facilities, member homes, or environments involving medication access.
• Candidates should be located within a reasonable commuting distance of a PulsePoint location.
